First Choice Pediatrics bills all major insurance plans. Therefore, we will accept the insurer’s approved amount for covered services as our fee for all carriers with which we have a signed contract. You are responsible for co-insurance, co-payments, deductibles, and non-covered services.
All co-pays must be paid prior to your office visit, if you haven’t paid your co-pay prior to your visit, then you will have to reschedule your appointment.

We recommend that you review your insurance policy and contact your insurance company before your exam to determine which services are covered by your insurance plan, and what information must be provided to your insurance company.
Please bring your insurance identification cards or referral forms with you when you visit our office. We will be happy to answer any questions you have at that time.
As a courtesy to our patients, we will fill out and submit a standard AMA 1500 insurance claim form to your insurance company. It’s the patient’s responsibility to ensure their account is paid in full within ninety days; otherwise the account will be referred to a collection agency that can possibly affect your credit. If your insurance plan requires that you make a co-payment, that amount will be collected in the office at the time of the visit.

Financial Hardship
Eligibility for a sliding fee discount program is available in a few of our locations. It is determined by documented annual income and family/household size. Patients with a documented annual income of 200% or below the federal poverty level (FPL) are eligible for a sliding fee discount on all First Choice Pediatrics services.
Patients with incomes at or below 100% of the federal poverty level (FPL) may be charged a nominal fee; however, no patient will be refused services due an to inability to pay.
Determining your eligibility for the sliding fee discount requires verification of your income. This information must be updated frequently to continue your participation in the program. This information is only used to calculate your discount and is kept completely confidential.
